Hope ReStored Online Client Voucher Request

This request form is for approved Hope ReStored partner organizations only. Any other submissions will be disregarded.

This form must be completed accurately and entirely. If possible, we recommend that you complete this form with your client to ensure accuracy. The information provided through this form will be utilized to consider the request and to provide reporting that allows Hope ReStored to exist and thrive.

Once approved, a Hope ReStored gift card will be loaded for the requested amount and available for pickup at ReStore in Canton. Gift cards can only be collected by the person(s) named on this form. Hope ReStored gift cards can be picked up any time ReStore is open, Tuesday-Saturday from 10am to 6pm. Clients should allow one (1) business day to process the request before picking up their Hope ReStored gift card. Requests made on Friday will be available for pick-up the following Tuesday. Gift cards can be used immediately once collected.
